Five-Year Data: Surgery Betters Medical Therapy for Diabetes (Part 1)



Bariatric surgery is more effective than medical treatment alone for the long-term control of type 2 diabetes in obese patients, finds the first 5-year follow-up study, prompting a call for a rethinking of the diabetes care pathway with more emphasis on surgery.


Overall, 50% of patients who underwent surgery showed sustained remission of type 2 diabetes (defined as HbA1c less than 6.5%) without any medication upon 5-year follow-up, compared with none in the medically treated group.


"These people had nondiabetic glycemia for 5 years without ever taking a single antidiabetic drug," said Francesco Rubino, MD, senior author of the study and chair of bariatric and metabolic surgery at King's College London, United Kingdom.


And over 80% of surgically treated patients maintained the American Diabetes Association's (ADA's) treatment goal of HbA1c concentration below 7.0%, with just diet and/or metformin.


"This is a spectacular result," added Dr Rubino, although he acknowledged that the study involved only a small number of patients and was conducted at a single center.


The results were published in the September 5 issue of theLancet by Geltrude Mingrone, MD, of the Catholic University of the Sacred Heart, Rome, Italy, and colleagues, including Dr Rubino.


Like Cancer Treatment, Surgery Should Be Option in Diabetes


In an accompanying editorial, two other bariatric surgeons, Dimitri Pournaras, PhD, MRCS, and Carel le Roux, MBChB, PhD, both from Imperial College London, United Kingdom, draw a parallel with multimodal cancer treatment, where the best surgical treatment combined with the best medical treatment is better than either alone.


"This model could be followed for diabetes, with use of best medical care to maintain remission, which is often only possible with surgery," they write.


They suggest that future randomized controlled trials should assess the optimal point at which to use surgery, but that "all surgical groups should receive intensive medical treatment and close follow-up at some stage because, as [Dr] Mingrone and colleagues have now shown, not doing so results in relapse of diabetes."


Dr Rubino concurred, noting that the management of type 2 diabetes could come to resemble that of cardiovascular disease, with treatments ranging from diet and exercise, to drugs, to endoluminal stents or surgical bypass.


"Introducing surgical treatment in diabetes management allows escalation" of therapies according to disease severity and enables risk stratification, he told Medscape Medical News in an interview.





"If you know that surgery is an option, you give diet and drugs a reasonable time to work; if they fail, surgery should be considered," he added.

Type II DM Control after Bariatric Surgery (1)

Type II diabetes is currently one of the fastest growing epidemics in known human history. It is generally agreed to be closely associated with obesity, with various clinical manifestations. Besides, multiple risks factors are involved in the causation of diabetes.
Moreover, diabetes has a progressive nature with no cure at present. Disease control is performed by continuous monitoring and re-assessment of diabetic management measures.
Prognosis of the disease usually leads to intensification of therapy with increasing dose of OHA, including insulin, according to the generally accepted algorithms provided by American Diabetes Association. [1]


ADA diabetes treatment algorithms suggested lifestyle modification for patients at risk of diabetes. Once patients are diagnosed, metformin mono-therapy is the recommended first line treatment, followed by additional second-line medications if HbA1C target levels has not been achieved. Eventually, it will be the use of insulin.


The application of bariatric surgery in diabetic control has been studied and its effectiveness is compared against conventional treatment, such as intensive medication therapy. Patients seemed significantly benefited from bariatric surgeries in combination with medication therapy.


Bariatric surgery VS intensive medication therapy - STAMPEDE Trial (Surgical Treatment and Medications Potentially Eradicate Diabetes Efficiently)

In a randomized, non-blinded, one-center trial, the efficacy of intensive medication treatment was compared with the combination of medication and bariatric surgery, either Roux-en-Y gastric bypass or sleeve gastrectomy in 150 obese patients with uncontrollable type 2 diabetes. The primary end point of the trial was the proportion of patients with glycated hemoglobin level (HbA1c) 6.0% or less at 12 months after treatment. Moreover, change in fasting plasma glucose, average numbers of diabetes medications and change in BMI were the second end points of the trial.


It is concluded that 12 months of combined treatment of medication and bariatric surgery could significantly achieve glycemic control than medication alone, as only 12% of patients were able to reach the first end point, versus 42% in the gastric bypass group and 37% in the sleeve-gastrectomy group. The results of the other end points were shown in Figure 1.

肥胖

漲卜卜的大肚腩、儼如米芝蓮的肥胖身形,若你認為這是有「福氣」的表現,這便大錯特錯。肥胖﹝Obesity﹞不但影響健康,更與其他慢性疾病息息相關,當中二型糖尿病及代謝綜合症,便是因肥胖而引致。目前醫學界有各種適合不同患者需要的微創減重手術,透過限制食量,改變吸收系統,以全面性方法達至減重效果。

何謂肥胖?

肥胖是因身體儲存過多脂肪,這與吸收過多熱量,以及熱量消耗不足所致。不少市民因暴飲暴食、愛吃高脂肪飲食,以及缺乏運動,令身形愈見肥胖。要知道體重是否過高,可從計算體重指數(BMI)得知,BMI為國際公認成年人肥胖程度的指標,BMI計算程式為:體重(公斤)÷〔身高(米)x身高(米)〕。

為何要治療肥胖?

一副圓碌碌的身形,不單純是外貌,穿不上「靚衫」的問題,更嚴重的是,過重會引致很多慢性疾病,影響健康,甚至致命。一般來說,肥胖與糖尿病、高血壓、心臟病、足踝退化、睡眠窒息、高血脂等有關,而女士的經期不準或荷爾蒙分泌問題等,也與肥胖息息相關。另方面,中央肥胖更是代謝綜合症的主要成因,大大增加患上中風、心血管疾病及糖尿病的機會。相較西方國家,亞洲人出現中央肥胖情況更較多。

至於同樣較多亞洲人患上的糖尿病,亦絕不可輕視,此症會引發很多併發症,例如青光眼及心臟病等。不過,不少糖尿病患者也忽視肥胖的問題,認為過重不是疾病,很多時在服藥及注射胰島素後,仍久久未能抑制血糖過高,才醒覺肥胖的問題性,在家庭醫生轉介下,向專科醫生求診進行微創減重手術。

若BMI達25至29.9,這屬於中度肥胖,在30或以上則屬嚴重肥胖。根據亞太代謝性疾病和肥胖外科學會(APMBSS)建議,亞洲人BMI達32並同時患上糖尿病或其他兩種有關肥胖的疾病例如高血壓,或BMI超過37以上的患者,可能有需要進行微創減重手術。






各種減重的方法:

運動及減少食量是基本減輕體重的方法,部分人士在合適情況下,可經醫生處方藥物減肥。不過,若年齡為18至65歲,在服藥、經營養師指導下節食,以及運動後,仍不能持久減輕體重,而BMI又超過37以上,或BMI達32並同時患有糖尿病或其他兩種有關肥胖的疾病,便可嘗試進行微創減重手術,包括腹腔鏡胃束帶手術、腹腔鏡縮胃手術、腹腔鏡胃摺疊手術及腹腔鏡胃繞道手術。另透過內視鏡在胃內置入水球,亦同樣能控制體重。若18歲以下患者,經醫生檢查後情況許可,亦可進行以上手術。
